Please complete and submit this form to nominate an individual or organisation for one of our best practice awards. Judging will take place after the closing date for nominations and we will contact you then for further information if your nomination has been shortlisted.
Please be specific about how the person or organisation meets the judging criteria. We suggest a word limit of 400 words for this section of the nomination form and the box will get bigger as you type.
Experience shows that organisations and individuals have a higher chance of being shortlisted if we receive a variety of supporting information from customers, employers and other stakeholders. You can submit further evidence by uploading a single pdf to the form below or using a hard copy nomination form. It always helps to include data evidence such as job outcomes achieved and sustained.
